IMPORTANT NOTICE
Brookside Pediatrics is closed, effective December 31, 2022.
As of that date, Dr. Harold Cronin is retired from practice, and the office will be permanently closed for appointments.
For a limited time, medical records will be available via the Athena
Portal. You can sign on to the patient portal as you have in the past.
If you cannot access the portal to print your patient records, you may
be able to get them from our office by following these directions:
1) Download the Patient's Record Release Form below, and print a paper copy.
2) Get the signed, completed form to our office at the address above. Be sure that your new doctor's complete name and
mailing address are included.
You may hand deliver
the form, fax it, or mail it to our office. You may email the form to: brooksidepediatrics@charter.net
3) Our office will mail the patient's records to the doctor's office
designated on the form.
Note:
Many offices are currently NOT accepting new patients.
We have contacted Pediatric Health Care Associates (PHA). They will accept new patients with a signed Patient's Record Release Form from our office. They have 4 local offices.
We have also contacted Northeast Family Practice, located on Quarry Road in Trumbull. They will also accept new patients with a signed Patient's Record Release Form from our office.
